29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place: The employer is failing to protect employees from struck-by hazards while operating powered industrial trucks. This was most recently documented on or about 5/31/17 at the establishment located at 515 S. 4th Street in Osage City, Kansas. Employees working in close proximity to powered industrial trucks throughout the facility were exposed to struck by hazards in that all nameplates and markings were not in place on the following forklifts: (b) Nissan 40 - data plate is damaged and not illegible and the Direction Control Lever is not labeled.
