Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Production - On May 25, 2017, the employer did not ensure that one or more methods of machine guarding was provided to protect employees from hazards such as those created by in going nip point and rotating parts on the cubix counter ejector at machine #3141. (b) Production - On May 25, 2017, the employer did not ensure that one or more methods of machine guarding was provided to protect employees from hazards such as those created by in going nip point and rotating parts on the ejector area at machine #3111. (c) Production - On May 25, 2017, the employer did not ensure that one or more methods of machine guarding was provided to protect employees from hazards such as those created by in going nip point and rotating parts on the scrap belt at machine #4562. (d) Production - On May 25, 2017, the employer did not ensure that one or more methods of machine guarding was provided to protect employees from hazards such as those created by in going nip point and rotating parts on the stacking deck at machine #4562. Westrock CP, LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.212(a)(1) which was contained in OSHA Inspection Number 1177572, Citation number 1, Item number 2 and was affirmed as a final order on November 21, 2016, with respect to the workplace located at 975 N. Freedom St., Ravenna, OH 44266. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.
