Text For Citation: 01 Item/Group: 001 Hazard: CONFINED
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Oxygen deficiency inside the diffusion pump of furnace #2 on or about 6/8/92. Maintenance employees were not adequately trained in duties and general hazards of confined space entry and rescue prior to any confined space entry. One feasible and acceptable abatement method, among others, to correct these hazard(s) is: 1. Training employees on duties and hazards of confined space entry and rescue. Employer may reference training requirements in ANSI Z117.1-1989 Safety Requirements for confined space, NIOSH No. 80-106 Working in Confined Spaces, or OSHA proposed rule 1910.146 Permit Required confined spaces. 2. Written procedural steps for confined space entry and rescue. 3. Atmospheric testing prior to entry.
