29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: On and about May 12th, 2017, an employee engaged in construction activities from an elevated catwalk was exposed to fall hazards of approximately 15 feet to the next lower level in that a floor hole in the catwalk was not covered.
