29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about May 12, 2017, on the south east side of the work site, the operator of a KUBOTA SVL95-2s skid steer loader was not trained to lower the attachment and stop the engine prior to getting out of it and was exposed to caught in or between hazards.
