Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: Worksite: On/or about March 26,2003. The employee who was in the process of loosening up the chains/come alongs for the load/vacuum machine on the tractor trailer bed, while standing on the ground in between the tractor trailer and the wheel mounted mobile crane, was exposed to the hazard of being crushed or caught-in-between due to the following deficiences: 1. The brake for the truck was not set properly causing the crane to move toward the trailer. 2. The wheels were not chucked. 3. Follow the mobile crane standards paragraph 5.14 (means shall be provided to adequately hold crane stationary during working cycle. 4. Other useful methods. Consequently, the crane rolled foward pinning the employee between the crane and the trailer causing severe injuries.
