The outriggers were not firmly and securely placed on the surface of the ground or firmly and securely on the industry approved outrigger pads which were firmly and securely placed on the ground.. 3. The wheel chocks were not placed under the wheel at a minimum of two (2) when conditions warrant, preferably under the rear two (2) wheels. Among other methods, one feasible and acceptable method to correct the hazard is to: 1. Evaluate the surrounding areas and existing conditions prior to parking and setting up the truck(s). 2. Place the outriggers firmly and securely on the surface at all times. 3. Place more than one (1) wheel chocks under the preferred wheels at all times. 4.
