Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)The hazard of being crushed or struck by a 1 inch X 8 feet X 20 feet steel plate, weighing approximately 6,520 lbs. lifted with a alloy steel sling with a sorting hook which was lifted by a CAT 345B excavator. An employee was fatally injured on or about December 8, 2000 when the hook came loose from the steel plate which struck the employee. One feasible and acceptable abatement method, among others to correct this hazard would be to use wedge clamp as referenced in National Safety Council, Data Sheet 565, Reaffirmed 1982.irmed
