29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed. a) QC Line 2 - A rotating sprocket wheel and chain on the automated conveyor used to move boxes down the conveyor line was not guarded. The tips of an employee's two fingers were amputated when they attempted to retrieve jammed boxes from the conveyor line, on or about 4/28/17. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
