Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: WORKPLACE: ON OR ABOUT MARCH 27, 1999 AN EMPLOYEE DROVE A E-Z-GO ELECTRIC POWERED CARGO AND PERSONNEL CARRIER 875E, MODEL XI 875, SERIAL NUMBER 1157981 UNDER THE FRONT OF A DISCONNECTED HYUNDAI VAN TRAILER MODEL VI25300152JS. THE EMPLOYEE HAD NOT RECEIVED TRAINING ON THIS PIECE OF EQUIPMENT AS REQUIRED BY E-Z-GO OWNERS MANUAL AND SERVICE GUIDE (MANUAL: 34674-GO1) AND ASME/ANSI B56.8.
