Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to being struck or hit by loads and/or crane parts due to "hoist failures": MILL: The following jib hoists for the equipment listed below had the following deficiencies: 1. 1-ton hoist for the 80 billet Hem structural saw: a. A piece of copper wire was wrapped around the throat of the hook as a "safety latch" of the hook. 2. P&H 1-ton for the Warner & Swasey lathe: a. A small piece of metal was welded on across the throat of the hook as a "safety latch" due to defective safet latch of the hook. 3. P&H 1-ton for the #14 hydrotester: a. The pendant control was not marked for the "down" position. b. The wire rope was not properly reeving/winding on the drum. c. One of the 2-part wire rope had two (2) bent sections. 4. Beacon Line crane/hoist for the Ultrasound machine: a. The capacity marking on the jib/beam did not match with the marking on the load block. b. One of 2-part wire rope was bent in one (1) section. 5. The marking on the beam of the Eastern A-frame mobile hoist was worn out. Among other methods, one feasible and acceptable means to correct the hazard is to: 1. Follow the manufacturer's recommendations. 2. Comply with ANSI/ASME B30.16-1993; Overhead Hoist (Underhung) guidelines for inspections, maintenance, and operations of the overhead hoists, Chapter 16-1, sections 2.1 to 2.6..
