29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: (a) Employees working on and around the Case Packer were exposed to a fall hazard of approximately 10 feet. The employer did not ensure that a guardrail extended around the entire balcony edge/side where the Case Packer was located. (b) Employees using the stairway to access the office where exposed to a fall hazard of approximately 10 feet. The employer did not ensure the upper landing of the stair way was properly guarded on all open sides.
