Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to multiple broken bones or death: JOBSITE: A job made work platform from which an employee was working and placed on the forks of a JCB Model 508C rough terrain forklift to access a small window roof was inadequately secured to the forks or frame of the forklift to prevent forward movement. Among other methods, one feasible means of abatement to correct the hazard is compliance with the American National Safety Standards (ANSI) B56.6-1992 "Safety Standards for all Terrain Forklift Trucks", chapter 5, section 5.1 "Elevating Personnel".
