29 CFR 1910.137(c)(2)(xii): The employer shall certify that equipment has been tested in accordance with the requirements of paragraphs (c)(2)(iv), (c)(2)(vii)(D), (c)(2)(viii), (c)(2)(ix), and (c)(2)(xi) of this section. The certification shall identify the equipment that passed the test and the date it was tested and shall be made available upon request to the Assistant Secretary for Occupational Safety and Health and to employees or their authorized representatives: On or about 08 May 2017, and on dates prior, the employer did not ensure that the rubber insulating gloves issued for employee use had been tested within the past six months since being placed into service to ensure protection at the rated voltage. Certification was not made available to the Secretary's representative upon request. Employees were exposed to electric shock hazards when performing electrical work. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
