29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about May 5, 2017 in the maintenance shop, lockout and tag out procedures were not developed, documented, or utilized where employees work on forklifts and battery chargers, employees are exposed to hazards to include, but not limited to : a. a crushing hazard when working at removing batteries and changing tires b. a freezing hazard when changing propane cylinders when hoses have to be repaired or replaced c. electrical hazards of up to 220V when working on the battery chargers
