Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. A.) On or about April 30, 2017, employees of Pennsylvania Power Company were exposed to roll-over / crush hazards when operating an Altec AT37-G aerial lift on a Ford F-550XL Super Duty truck chassis on an inclined roadway that exceeded a five degree slope. The truck rolled backwards and tipped over, causing the bucket to strike the roadway. ABATEMENT NOTE: Amongst other means, feasible means and methods to abate this hazard included, but are not limited to: - Follow the procedures listed in the Altec AT Class Operator's Manual which states to operate the unit on firm surfaces that are level or have up to a five degree slope. - Provide training to employees about the hazards of operating said vehicles on slopes greater than five degrees.
