29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: a) Jobsite: VIP Platform - The platform was not fully decked on all working levels exposing employees erecting the platform to fall hazards of up to 23 feet. An employee on the upper platform installing a 4 x 8 foot section of decking on a 4 x 8 foot deck opening, approximately 23 feet above ground level, fell through the opening to the ground level and suffered multiple injuries. The deck openings were used to access the understructure. Multiple hoists were run through these deck openings and connected to the understructure at the point during the erection process when the entire platform needed to be raised to install columns underneath. Noted on or about 4/22/17.
