29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section: a) 1804 W. Central Road, Mount Prospect, IL, 60174 - On or about April 21, 2017, the employer did not ensure equipment-specific energy control procedures were utilized for the control of potentially hazardous energy during testing for electrical contact to an overhead door operator in Bay #3, which resulted in multiple bodily and facial injuries to two service technicians that were exposed to a fall hazard of approximately 14 feet while on a scissor lift.
