29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4-feet (1.2 m) or more above a lower level is protected from falling by a guardrail system, safety net system, or personal fall protection system. On or about April 22, 2017, along the upstream side of the Hamilton Dam between Keokuk, IA and Hamilton, IL, employees were exposed to internal injury and drowning hazards and the employer did not provide suitable fall protection for the employees that were exposed to an approximate 10-foot fall hazard.
