Site - On or about April 13, 2017, the employer failed to provide guarding at the seven window openings on the second floor level where employees were exposed to falls of over 15 feet. Instance b. Site - On or about April 13, 2017, the employer failed to provide guarding at the basement staircase floor opening where employees were exposed to falls of over 7 feet.
