29 CFR 1910.22(d)(2): Hazardous conditions on walkingworking surfaces were not corrected or repaired before an employee used the walking-working surface again. If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired: a) On or about 4/24/2017 and at times prior thereto the employer did not ensure that a rectangular floor hole approximately 10 inches by 7 inches within the grain cleaning room located in front of the Newton grain cleaning machine was repaired and/or guarded exposing employees walking and working in this area to a tripping hazard. b) On or about 4/24/2017 and at times prior thereto the employer did not ensure that a rectangular floor hole approximately 14 1/2 inches by 7 1/2 inches within the grain cleaning room located behind the Newton grain cleaning machine next to the collection chute was repaired and/or guarded exposing employees walking and working in this area to a tripping hazard. c) On or about 4/24/2017 and at times prior thereto the employer did not ensure that two (2) floor holes approximately 6 1/2 inches by 4 inches oval within the grain cleaning room located between the Newton grain cleaning machine and the East wall were repaired and/or guarded exposing employees walking and working in this area to a tripping hazard. d) On or about 4/24/2017 and at times prior thereto the employer did not ensure that a circular floor hole approximately 5 inches in diameter within the grain cleaning room located behind the Newton grain cleaning machine was repaired and/or guarded exposing employees walking and working in this area to a tripping hazard.
