29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 12, 2017, an employee were exposed to avulsion and amputations while operating an Amada Press Brake Model Number FBD8020NT, Serial Number 80203162, without machine guarding to protect the employee from the point of operation hazards.
