Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Outside Yard of Majorsville Field Office; Improper couplings were used on a pipe which was approximately 38 feet long. During a pressured air test, one of the couplings released the end cap, the pipe jumped and the employee working beneath the pipe was killed by the jumping pipe and it's wooden stand. (b) Outside Yard of Majorsville Field Office; An employee was beneath a 38 foot long gas pipe which was approximately 42 pounds per foot. The round pipe was on square 6" x 6" wooden blocks approximately 36" long. The round pipe was not attached to the 2 piles of square blocks that were not attached to each other. The failure of an operation (charging the pipe with air pressure) caused the pipe to jump, it thereby displaced the wood and an employee was consequently killed when his head was crushed.
