29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee was protected from falling through any hole that was 4 feet (1.2 m) or more above a lower lever by one or more of the following: On or about March 28, 2017, and times prior thereto, Soy Division Preparation building, the metal grated floor and drag conveyor hole openings located on the 3rd floor on the north side of the mill, were not covered or guarded. Employees were exposed to unprotected falls of greater than 4 feet through the hole openings to lower levels.
