Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee in that employees were exposed to: a) The hazard of not being able to control the well in the event of a kick or blowout due to the failure of Farrar Oil Field Service Equipment Co., Inc.'s crew of workover rig #4 to ensure that well blowout prevention equipment would sufficiently control pressure from any impending eruption(s) of gas and/or fluids from the R. & H. - Travis #3 Well located in Section 6, TION R9W, Japer County, MS. This occurred on or about Tuesday, May 29, 1990. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to conduct inspections and operating pressure test(s) of blowout prevention equipment, including but not limited to blowout preventer(s) and T.I.W. valve(s), and note the results of such tests/inspections, accordance with A.P.I. RP-53: Recommended Practices for Blowout Prevention Equipment Systems for Drilling Wells, Second Edition, May 1984, and paragraph 4.4.2 of A.P.I. RP-54: Recommended Practices for Occupational Safety and Health for Oil and Gas Well Drilling and Servicing Operations, First Edition, January 1981.
