Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to amputation and crush-by hazards from damaged dock leveling systems: On or about April 20, 2017, employees receiving cargo were exposed to crush-by injuries, broken bones, and amputations when operating a damaged dock leveling system which had a broken extension spring and the employer did not remove the damaged dock leveling system from service. Among other methods, feasible and acceptable means of hazard abatement include: (1) To correct this hazard comply with the manufacturers safety, inspection, and operational instructions for end dock leveling devices.
