29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: A. On or about April 3, 2017, Outside, Behind Plant: An employee, transporting a decommissioned metal tank to a recycling roll-away dumpster, sustained a fingertip pad amputation injury when the tank the employee was guiding slipped. The amputation injury was reported to the OSHA office on April 18, 2017.
