29 CFR 1910.23(b)(10): (10) Any ladder with structural or other defects is immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with �1910.145 and removed from service until repaired in accordance with �1910.22(d), or replaced; a. At the workplace in the Receiving Area, a Tri-Arc Manufacturing Co. steel rolling ladder had approximately four inch wide holes in the ladder's bottom two treads, exposing employees to trip and fall hazards while ascending and descending. Employees were using the ladder to access merchandise stocked on the upper shelves.
