The employer did not ensure each flight of stairs having at least 3 treads and at least 4 risers was equipped with stair rail systems and handrails per Table D-2; stair width less than 44 inches (1.1 m), enclosed, did not have at least one handrail. On or about April 17, 2017, and at times prior to, the employer did not ensure that the stairs ascending into the blast room had at least one stair rail.
