29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: a) On or about April 6, 2017, when the employer did not ensure that employees performing maintenance work on OSSID machine #9 utilized energy control procedures developed for that equipment, exposing employees to amputation hazards.
