29 CFR 1910.106(d)(7)(iii): Open flames and smoking were permitted in flammable or combustible liquid storage areas: In building #15 employees were exposed to fire and explosion hazards in that open flames were allowed to be used to heat and clear a blockage in a line, causing the vapors of approximately 2 gallons of isopropyl alcohol to ignite forcing employees to evacuate the premises from the uncontained fire.
