29 CFR 1910.119(d): The employer did not complete a compilation of written process safety information, including information pertaining to the hazards of the highly hazardous chemicals used or produced by the process, information pertaining to the technology of the process, and information pertaining to the equipment in the process before conducting the process hazard analysis: The employer is failing to protect the employees form the hazard of explosion by failing complete a compilation of process safety information for the covered processes. This was most recently documented at 25201 East 78 Highway, Independence, Missouri when an employee was fatality injured while breaking up Tetrazene inside the vacuum receiver The process uses a spatula and the company failed to compile information on the material of construction and how this information may impact the use of the spatula with tetrazene.
