29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them: a) Costco Warehouse (Dairy Department): An employee working from the platform of a Genie scissor lift was required to perform electrical work on a ballast, rated for 440 volts, that was not deenergized. The employer did not ensure that the electrical system was deenergized prior to allowing the employee to perform work, thus exposing the employee to an electrical shock hazard, on or about, 4/10/17.
