29 CFR 1926.850(a): Evidence in writing was not available that an engineering survey performed by a competent person had been performed prior to permitting employee(s) to start demolition operations: a) At the jobsite, in the boiler room, on or about 4/6/17: The employer did not ensure that an engineering survey was conducted prior to cutting out sections of 4 inch pipe, hanging from the ceiling; that would prevent unintended collapse of that pipe. Workers were exposed to struck by hazards when a section of pipe fell through hangers and struck one employee, causing him to fall from his ladder. This employee received multiple injuries and was hospitalized as a result.
