29 CFR 1910.212(a)(2): Guards were not affixed to the machine where possible, or secured elsewhere when attachment to the machine was not possible. Machine guard on machine was so installed that it offered accident hazards in itself. (a) Machine guarding: On the 54 inch slitter, a piece of steel tubing was put in place to use as a "guard bar" to insure that the overarm would not go down far enough to make contact with the machine spindle during setup or during maintenance. The guard bar was installed improperly (upside down) which created an amputation hazard between the overarm and the machine spindle.
