Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: A. In the following instances, lockout procedures were not utilized to control the unexpected startup of equipment during operations, when the safety guard of the equipment was removed: 1. On or about April 5, 2017, Oatmeal Line: An employee engaged in purging oatmeal from the conveyor to prepare for a new flavor, opened an interlocked access door at the base of the conveyor and reached into the opening. The interlock failed and the auger conveyor started unexpectedly, causing an amputation injury to the index, middle and ring finger of the employee's left hand. 2. On or about April 12, 2017, Oatmeal Line: A maintenance employee repairing a damaged air cylinder did not lock out the equipment following the company's general lockout procedures. No machine-specific lockout procedures had been developed for the equipment. Lockout hardware was not utilized to ensure that cord and plug connected equipment would not be energized while out of the authorized employee's control.
