On or about March 24, 2017, an employee was admitted to the hospital with an injury from a construction site accident and on March 30, 2017 as a result of the extent of the accident injury the employee's left leg below the knee was amputated, and the employer did not report the work-related injury hospitalization to OSHA within 24 hours.
