Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of tank rupture, explosion or release of highly pressurized water. North bay, West end of building at 1000 West 142nd Street, Riverdale, IL.; on or about 10/23/98, employees using uncalibrated impact wrenches to tighten 24 12-inch studs on both the M-1 and M-2 flange joints of the Vacuum Fat Splitter could not determine the bolt load placed on the studs. A feasible and useful means of abatement includes, but is not limited to the following actions: 1) Establish bolting procedures, train employees to utilize those procedures, including ensure their understanding of the advantages and limitations of using torque wrenches, bolt tensioners, bolt heaters and impact wrenches to tighten studs and bolts, and use torque wrenches that have been calibrated. 2) Determine the torque load prior to torquing in accordance with the gasket manufacturer's recommendations, as found in the Torgue Table for Spiral Wound Gaskets used with the American National Standard Institute's (ANSI) 16.5 Flanges standard.
