Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Location: Warehouse Yard; employees were exposed to the hazard of being struck and/or crushed between a tractor trailer and loading dock. On 12/16/94, an employee was seriously injured while adjusting railroad ties in the rear of a tractor trailer prior to the truck driver backing the trailer over the railroad ties. Among other methods, one feasible and acceptable abatement method is to: Establish and implement the following written safety procedures for tractor trailer drivers to follow whenever backing towards a loading dock such as: 1. Observe the whole area before backing; 2. Use an observer whenever possible; 3. Do not start backing until the rear is clear; 4. Back slowly using the hazard lights; 5. Use both side mirrors at all times; 6. Whenever in doubt, get out and observe the whole area again; 7. Give signal by blowing horn several times;
