29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: A) Metal Conversions, Ltd., located at 849 Crawford Avenue, Mansfield, OH 44905: On or about March 21, 2017, the employer did not evaluate the workplace in order to determine if the bailer (manufactured by Harris Press & Shear, Inc., Model HRB8) was a permit-required confined space. Hazards, associated with a permit-required confined space, were not identified before employees entered the bailer in order to conduct activities such as, but not limited to, compacting the material. Employees were exposed to the hazards associated with the bailers operation (amputation hazards).
