Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees : a) Wells Fargo Bank Vault, Cheltenham Mall - On March 27, 2017 an employee was fatally crushed by a safety deposit box unit that fell as it was being lifted by two manually powered hydraulic lift stackers. The weight and dimensions of the safety deposit box exceeded the manufacturer's specifications for the two hydraulic lift stackers used to carry the load. Among other feasible means of abatement would be: 1. Ensure that lift condition, use, and employee training comply with all elements of ANSI/ITSDF B56.10-2012 Part II, section 4 General Safety Practices and section 5 Operating Safety Rules and Practices. 2. Ensure any load placed on a lift is within size, weight, and load limits prescribed by the manufacturer's manuals, data plates, and warning labels. 3. Conduct a Job Hazard Analysis (JHA) as specified for non-routine tasks to identify and eliminate employee exposure to hazardous conditions and operations and ensure selected equipment is suitable for assigned tasks.
