Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a)In department 03 at the monorail load station, there was nothing to protect load station operators in the event that a casting would fall into the employees' work area, thus exposing employees to a struck-by/crushed-by hazard while loading castings on the monorail system. On 5-23-96, a monorail load station operator was seriously injured when a frame at the north load station fell out into the operator's work area. Among other methods, two feasible and acceptable abatement methods to correct this hazard are the following: 1)Install a barrier guard at the north and south monorail load stations to prevent castings from falling out into the operator's work area. 2)Relocate all operator controls to ensure that the operator will remain in a safe location before castings are raised and while castings are in an upright position at the monorail load stations.
