Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to injury from unintended actuation of machinery: a) Shear Department - The foot-operated switch on the BUFFALO shear was not protected so as to prevent accidental actuation by falling or moving objects and from unintended operation by accidental stepping onto the switch. b) Forge - The foot-operated switch on the BLISS pneumatic press at Hammer station 14 was not protected so as to prevent accidental actuation by falling or moving objects and from unintended operation by accidental stepping onto the switch. ABATEMENT NOTE: Among others one feasible and acceptable method to abate this hazard is to cover the foot pedals.
