The employer fails to assure effective protection against falls as required by the standard, as demonstrated on March 27, 2017, when employees working on a 3 story residential, 5:12 pitch steep slope roof at 2898 Ocean Shore Blvd, Ormond Beach, FL 32174 are not protected from falling 20 feet to the ground below by any means of personal fall protection, guardrails, safety nets, or any other type of fall protection. Oviedo Roofing Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(B)(11), which was contained in OSHA inspection number 924090, citation number 1, item number 1, and was affirmed as a final order on September 3, 2013, with respect to a workplace located at 800 N. Highway 27, Mineola, FL 34715.
