Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or about January 29, 1996, an employee injury resulted when a section of 1 and 1/2 inch thick steel plating, 40 by 45 inches, and weighing approximately 748 lbs. slipped out of the crane hook, striking an employee in the leg. The employer did not inspect, document, and have removed from service the jib crane's block and hook as specified in the American National Standards Institute (ANSI) sections B.30.16-Overhead hoist, B.30.11-Monorail systems and Underhung Cranes, and B.30.10-Hooks. Among other methods, one feasible and acceptable method to correct the hazardous condition is to replace the Crosby-Laughlin Hook with one meeting the ANSI specifications mentioned above.
