Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that on and before October 31, 1995, employees were exposed to the hazard of broken bones, lacerations, internal injuries and or death from operating the coil car at the McKay slitting system where a worker would be in the hazard area created by the movement of the coil car and adjacent peeler machinery while repositioning the inside tail of the steel coil as it was being moved into position on the mandrel. Communication was not maintained and the coil car operator could not keep auxiliary operating personnel clearly within his field of view. Methods of abatement may include, but are not limited to: Ensure implementation of ANSI B11.18--1992 Machinery and Machine Systems for Processing Strip, Sheet, or Plate from Coil Configuration--Safety Requirements for Construction, Care, and Use. (1)Eliminate hazards or risks by design such as moving the coil car controls to a central elevated location or moveable remote control where hazard areas can be continually monitored for personnel (per Section 5.4). (2)Provide safeguarding against hazards associated with the combination of individual fixed and moveable machinery by guards, presence sensing devices, awareness barriers and or warning means (per Section 5.7). (3)Train and enforce employees to operate, maintain, and adjust the machine or systems in a safe manner and instruct them in the procedures necessary to avoid injury including the care and use of safeguarding (per Sections 4.4.2, 4.5.2.1, 5.4 and 5.7.5).
