29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) 420 North 9th Street, Reading, Pennsylvania - On March 3, 2017, the employer failed to report the hospitalization of an employee within 24 hours for work-related gunshot wounds to the right arm and left lower abdomen. The employer reported this hospitalization to OSHA on or about March 6, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
