29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower level: a) Garage building: Employees were exposed to a 20 foot fall hazard when accessing the roof of the building that contained unprotected skylights in that an employee sustained severe injuries when he fell through a skylight and struck a truck frame and concrete floor, on or about March 15, 2017. Abatement Certification and Documentation Required.
