29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): The employer allows workers to be exposed to hazardous conditions created by not conducting adequate frequent or regular inspections. The most recent occurrence was at the new Bend Road Bridge construction project near 2301 Bend Road, Pacific, MO, where they failed to make frequent or regular inspections of the worksite to ensure all workers were in compliance with OSHA standards: a) Workers were exposed to a fall hazard from approximately 16 feet as they were working from an elevated work platform without personal fall arrest sytem; b) Workers were performing work over and adjacent to water without ring buoys readily available for emergency rescue; c) Workers were performing work over and adjacent to water without a lifesaving skiff immediately available for emergency rescue.
