29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: Tertiary ash hopper top, Boiler #1, 188 Main Street, Agawam, MA: On or about March 15, 2017 the employer did not ensure each employee was protected from tripping into or through a floor hole that was less than 4 feet above the next lower level while conducting ash removal activities by means of covers or guardrails. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
